Commercial Account Application

If you would like to apply for a Credit Account with Dultmeier Sales, simply print the PDF form, fill out, sign and fax or mail in. We will then e-mail your customer number to you; please allow 1-2 working days. Credit references may be requested. (You may always order any time by using your credit card or C.O.D.)

Tax Certification
Tax waiver form for purchases for wholesale, resale, ingredients or components of a new product or service; purchases to be resold, leased, or rented in the normal course of business. Please print, fill out, including your State Registration, Sellerªs State State Registration, Sellerªs Permit/ID Number of Permit, or ID Number of Purchaser. Then fax or mail in.

Streamlined Tax Certification
Alternate, streamlined tax certification form that can be used in these states only: Indiana, Iowa, Kansas, Kentucky, Michigan, Minnesota, Nebraska, New Jersey, North Carolina, North Dakota, Oklahoma, South Dakota and West Virginia. This certification is valid until cancelled by the customer or revoked by the state. Arkansa, Ohio, Tennessee, Utah and Wyoming will be eligible by 2008 (please call to verify).

Credit Card Authorization
Would you prefer to pay for your orders by credit card? To complete your request that Dultmeier Sales use your credit card to pay all the invoices for shipments on your account, please print, fill out, and fax or mail to us. We will keep your credit card information on file for purchases.

Dealer Eligibility
A questionnaire to establish the eligibility of a company as a legitimate resale dealer according to the definitions set forth by the National Distributor Associations.
